FCRA Applicant Rights Summary

This form provides your applicants with their rights, under the Fair Credit Reporting act, with regards to the hiring process and their background screening, Everyone who signs a consent form must receive a copy of this document.  This protects both the applicant and the potential employer.

Consent Based Social Security Verification (see links below, both forms must be used)
  1. Pre-Approval Form for Consent Based Social Security Number Verification (CBSV)

  2. Authorization for the Social Security Administration (SSA) To Release Social Security Number (SSN) Verification

With the consent of the SSN holder, CBSV can verify if the SSN holder’s name, date of birth, and SSN match SSA’s records. CBSV returns a match verification of “yes” or “no.” If our records show that the SSN holder is deceased, CBSV returns a death indicator. CBSV does not verify an individual’s identity.
